Advantages – Fat Transfer
- Safer than injecting permanent substances such as silicone, PMMA, paraffin, or having implanted devices such as a silicone sleeve
- Liposuction is part of the procedure, removing unwanted fat from the stomach

Generally Less Safe (Penis Surgery is Higher Risk)
Fat transfer surgery is higher risk than penis filler injections due to the need to go under general anaesthetic, and a greater risk of complications. See recent death from fat embolism.
Fat is made by the body and found in most places, however, the body does put any fat in the penis naturally. The penis needs to be firm when erect rather than surrounded in a layer of fat.
Temporary – Usually lasts 3 – 12 months *
Fat is reabsorbed by the body over 3 – 12 months after which ‘top-ups’ are required. A small amount of fat can survive in the penis for a very long time, however, the result is not permanent.
Fat is living tissue and its survival requires it to establish a new blood supply after being removed from the stomach. There is no fat in penile tissue naturally, so the survival rate, when transferred to the penis is low. Any fat that survives is distributed arbitrarily along the shaft with areas of fat survival and areas in which no blood supply has been established (fat necrosis). The result is lumpiness.

Claims are often made that a fat transfer to the penis will provide a permanent result, however, this is misleading. A small amount of the fat transferred to the penis may survive for a long time, however, any permanent size gain is most likely to be in the form of hard nodules of necrotic fat with calcification. In almost all cases a fat transfer will need to be repeated a number of times in order to smooth out irregularities and maintain any increase in size.
Depending on your profession more than 2 weeks may be required off work and patients should not have sex for 6 weeks.
Fat transfer surgery typically takes 1-2 hours in a private hospital and typically does not require an overnight stay.

Infection (rare, but serious). Antibiotics are prescribed.
Skin fibrosis

Fat transfer is generally safe for penis enlargement, however, the results are inferior to dermal fillers.

- Risks of reversible dermal filler injections (low)
- Too much filler, the wrong type of filler, or filler injected poorly (too shallow), can give substandard results

Penis fillers are lower risk than surgery. No surgery is necessary and the procedure is reversible by injecting a substance which dissolves away the filler if necessary.
Penis filler is metabolised (absorbed) by the body over 12 – 24 months, at which point most patients have up to 50% of the filler remaining. In rare cases, penis filler can be absorbed more quickly, or last longer than expected.
High-risk substances, usually a form of polymer or plastic, can provide permanent results, however, they are not allowed to be used in USA as they cause serious complications.
In some cases, safe reversible penis fillers, and bio-stimulatory fillers can produce results lasting many months or years beyond their expected lifespan.
* As with fat transfer, it is possible to increase the girth of the penis by up to 2 inches, however, this requires a large number of injections.
More filler results in more penis girth.
